Insurance TeamPCP referral: Please review your insurance policy requirements to determine if you need a referral from your primary care physician prior to your visit. Call before you arrive to confirm that all needed referrals have been received.

Insurance information: Please remember to bring your insurance card to your appointment. Also, alert the office if your insurance information has changed and needs to be updated. Your insurance may require a co-pay for your visit, and this should be paid at the time of your appointment. If you are covered by an insurance company that requires pre-certification and authorizations for procedures or other services, please inform our office so that we can assist you.

The Neurology Clinic has enrolled in numerous managed care insurance programs to accommodate the needs of our patients. With each insurance program, there are many individual requirements of the plans having different stipulations regarding what services are covered and how often they can be performed. We encourage you to call your insurance company to determine if we are in your specific network and that the services we provide are covered under your plan. A list of some of the insurance plans we accept is provided for your convenience. This is a partial list and your insurance company or plan may not be included in this list.
